各省、自治区、直辖市及新疆生产建设兵团安全生产监督管理局，各省级煤矿安全监察局：
《安全生产举报信息统计报表制度》已经国家统计局同意，现予印发，即日起实施，有效期至2018年12月。 2013 年 2 月 19 日 印发的《安全生产举报信息统计报表制度》(安监总统计〔2013〕17号)同时废止。
